If you need ongoing care

  • Unlimited visits throughout the year
  • Prescription management and renewals
  • Lab ordering, monitoring, and result review
  • Priority scheduling — often same day or week
  • Extended physician access between visits

Enrollment fee: $200 one-time, non-refundable

Fee-for-service visits are available for patients needing only 1–2 visits per year ($550+). Membership is required for ongoing care and complex endocrine management.

Cancellation policy

If you’re unable to keep your appointment, please notify us as soon as possible so we can offer the slot to another patient — we’ll help you reschedule quickly. A no-show is defined as failing to cancel or reschedule at least 48 hours before your visit, and is subject to a $100 no-show fee. More than one no-show may result in dismissal from the practice, at the physician’s discretion.

We don’t participate with any insurance plan, and Dr. Rivera is opted out of Medicare. Our services are billed directly to you and cannot be submitted to Medicare or insurance for reimbursement.
