Whether in person, over the phone, via telemedicine, or through text, you’ll receive the personalized care you require, be it managing a single condition or multiple concerns. Membership Benefits

- Interpretation of data from glucose meters, continuous glucose monitors, and insulin pumps
- Remote monitoring for patients on insulin pumps and continuous glucose monitors
- Coordination of care with PCP and other specialists
- Access to Dr. Rivera in person, telemedicine, phone, email, or text

Your First Visit
Your initial consultation fee is $500. This includes:
Dr. Rivera’s comprehensive review of your complete medical history, labs, imaging, and other diagnostics
Your initial consultation, typically 60-90 minutes
Your first month of care, including a follow-up appointment to confirm diagnosis and develop, monitor, and adjust your treatment plan.
A non-refundable $75 booking deposit is credited towards the cost of this visit.
If you continue treatment at our practice, you have 2 options:
Enrollment in a Membership Plan, required when you need more than 2 visits per year and/or frequent communication with Dr. Rivera
If you only need 1 or 2 visits per year, you can simply pay per visit.

Membership Enrollment $200
- One-time, Non-refundable
*Membership is required for patients needing more than 1 or 2 visits throughout the year, as well as patients with type 1 or type 2 diabetes, or weight management. All memberships are billed monthly.
Diabetes in Pregnancy Membership $250/month
