Your First Visit
Your initial consultation fee is $495. This includes:
Dr. Rivera’s comprehensive review of your complete medical history, labs, imaging, and other diagnostics
Your initial consultation, typically 60-90 minutes
Your first month of care, including a follow-up appointment to confirm diagnosis and develop, monitor, and adjust your treatment plan if you decide you sign up for the membership plan
Your $75 booking deposit is credited towards the cost of this visit.
If you continue treatment at our practice, you have 2 options:
Our Membership Plan is the most economical when you need at least 1 visit every 6 weeks
If you only need occasional consultations, you can simply pay per visit

- Up to 8 visits per year (2 visits each quarter)
- Enhanced access to Dr Rivera including phone, text, and/or email
- Individual services such as prior authorizations, appeals, forms, letters, as well as CGM/Insulin Pump review
Membership Enrollment $300
- One-time, Non-refundable
*Membership is best for patients needing frequent visits throughout the year, as well as patients with type 1 or type 2 diabetes, or weight management. All memberships are billed monthly.
Diabetes in Pregnancy Membership $250/month
